MINOR WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to voluntarily participate in any way in st. George Maronite Catholic Church shooting seminar, the undersigned:

1.HAS READ AND UNDERSTANDS THE CONTENT OF THIS WAIVER AND RELEASE LIABILITY

2.Agrees and affirms that the minor participant is in good health and good physical condition and does not suffer from any disability that would prevent or limit their voluntary participation in basketball related activities.

3.Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence, but the action, inaction, or negligence of St.George Maronite Catholic Church/Buckner Fanning school (its employees, agents, directors, and owners ), and others, or the condition of the premises or any equipment used. Further, that there may be other risks not known to anyone or not reasonably foreseeable at this time. In addition, all photographs taken of participant become property of trinity performance sports and may be used in any promotional material.

4.Assume all the foregoing risk and accept personal responsibility for any damages or loss following injury, permanent disability, death or loss of property.

5. RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE St.George Maronite Catholic Church/Buckner Fanning School, their respective administrators, directors,agents coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises, all of which are hereinafter referred to “releases” from any and all liability to each of the undersigned, his or her heirs and next of skin for any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the “releases” or otherwise.
